Are you looking at form ZS-QU1? The way I understand this is that this form (at least the second page) is to be filled out or at least signed and stamped by some tax authority in your country of residence. You only need to do that if the relevant income is above 10.000 EUR. This is meant to be done beforehand so whoever pays you does not need to withhold taxes. The amount to fill in is the full income that would be taxed.</p>
